Valhalla Post Acute, also known as Old Oaklawn, is a skilled nursing and rehab facility in Louisville, KY. We have 162 beds in private and semi-private rooms, plus an expansive rehab gym
The Valhalla Post Acute Mission
Our goal is to enrich lives, one person at a time. It is a simple idea, and it is pretty hard to do well. For us, it means the highest-quality care we can give, in a safe environment, with the comforts of home.
A stay here usually comes at a hard moment, often right after the hospital. We’re here to be the step between the hospital and home, and a real home for the people who stay with us long-term. Our team reviews your progress every week and keeps you and your family in the loop.
Our Team
The people who work here are the reason families choose us. Our therapy team is in-house, and many of our therapists have been with us for years. A medical director and a nurse practitioner make rounds multiple days a week, so you’ll see a provider often.
Leadership
Our licensed nursing home administrator runs daily operations and staffing, with support from our department directors.
Therapy / Rehabilitation
Our physical, occupational, and speech therapists are in-house. They build a therapy plan around you and what you want to get back to.
Nursing / Clinical Staff
Our registered nurses assess and plan your nursing care. Certified nursing assistants are there to help with eating, bathing, and dressing.
Social Services & Activities
Our social services team looks after your emotional and social needs, and helps you keep as much independence as possible.
Dietary Services
Our registered dietitian oversees meal planning, so your meals are nutritious and fit what your body needs right now.
Environmental Services
This team handles housekeeping and laundry, as well as repairs throughout the building. They keep it clean and comfortable every day.

Seeing as how this is a nursing home I’m only going to tell you about the parking. Beware of the traffic circle. Also there are spees bumps. . if it’s cold outside and there’s snow or ice on the ground make sure to park in front of the building do not go down the hill. The landscaping is immaculate. The Halloween decorations are amazing. The groundskeeper does an expert job.
